All Required Forms

Please click the "2010 Required Forms" Link to the right to print and fill out all of the required forms that MUST be on file with Coach Beard before your child can participate in practice and/or meets.
There are three (3) forms. A Sport Physical, which must be completed by a health care provider, a Oldham County Board of Education permission form, and a Cross Country procedures form. If you get everything completed before school is out you may send it to the school office to the attention of Harmony Cross Country, Coach Beard.
